Physical Therapist Forms & Instructions


Physical therapy advisors, together with the Secretary of State’s Office of Professional Regulation (OPR), work to administer licensing for your profession. The documents on this page serve to guide applicants and licensees through initial application or renewal. Before you begin your initial, renewal, or reinstatement application, please review OPR’s application fees and 90 Day Initial Licensing Policy.

Fingerprint-supported background checks are required for physical therapist licensure in Vermont. If you have done a criminal background check for OPR physical therapist licensure and have not had a lapse in licensure you DO NOT have to complete another criminal background check. Learn more about Vermont’s new criminal background check requirements.

A 90-day provisional license can be issued if all application requirements are complete EXCEPT for the official verification of licensure or the results of a criminal background check. Apply online and request your 90-day temporary license today.

Continuing Education

Per H.305 (2023), where administrative rules require in-person or face-to-face continuing education, synchronous (live or interactive and not pre-recorded) virtual continuing education courses qualify as live, in-person training and will be accepted for renewal.
